Hammond Facilitation provides high-quality facilitation and consulting services related  to the California Regional Centers programs and services, including the California Self determination Program (SDP) as detailed in the Lanterman Developmental Disabilities Services Act.

Person-Centered Plan (PCP)

A PCP, tailored to the individual, employs person centred principles to reflect the client's and their support team's aspirations and objectives, as detailed in their Individual Program Plan (IPP). The PCP encompasses:

• Customized planning to meet the client's recreational, social, employment, or volunteer interests, along with enhancing community engagement, autonomy, health, safety, and future planning.

• Coordination and presentation of the PCP to the Client's regional center. • Support in developing the Client's Self-Directed Program (SDP) spending plan and securing a certified budget.

• The PCP is crafted collaboratively with the Client, their family, circle of support, service providers, regional center personnel, and relevant professionals

Transitional Services

Services include: 

• Assisting with applications for regional center services. 

• Helping clients transition from traditional regional center services to a customized SDP.

• Supporting the Client in moving from a family home to an independent living arrangement.

Self-Directed Supports (SD Supports)

SD Supports are designed for consumers  who have completed a SDP orientation and are considering enrollment in the SDP. SD  Supports provide assistance, coaching, and training in key areas to empower the Client in  managing their services and supports. These areas include: 

1. Individual Program Planning (IPP): 

• Collaboration with the professional who facilitated the initial PCP. 

• Support during the IPP meeting to articulate the Client's needs, goals, and preferred services within the SDP. 

2. Service and Support Coordination

• Guidance in identifying and hiring qualified personnel or agency providers. • Assistance with negotiating compensation and conducting staff criminal background checks. 

• Information on service provider qualifications, employment regulations, and staff management practices (e.g., setting expectations and hours of work, timely submission and claiming for hours worked). 

• Coaching on all aspects of staff onboarding and training procedures. 

3. Budget Management

• Education on leveraging generic funding sources to optimize SDP funds. • Help with obtaining a certified 12-month budget and developing an individual spending plan. 

• Training in basic budgeting and expense tracking skills. 

• Clarification on SDP fund utilization in compliance with state and federal legal and regulatory requirements. 

• Support in coordinating communication between the regional center, Financial Management Services (FMS) provider, and hired staff. 

SD Supports are tailored to ensure that the Client transitioning to or considering SDP are well prepared to manage their individual program plans, coordinate services, and effectively utilize  their budgets, thereby fostering greater independence and self-determination.

Consulting and/or Support for the SDP Process

Services include:

• Providing documentation support, offering guidance, and advocating for the Client's needs.

• Participating in necessary meetings.

• Liaising with regional center staff and third-party service providers.

• Gathering, organizing, and communicating information critical for developing the IPP and an initial PCP.

• Facilitating collaborative efforts among the Client, the Client's Team, family members, and regional center staff.

• Developing and writing the Client's IPP, a mandatory document used by regional centers to identify needs and services.

Ongoing Facilitation, Consulting, and Advocacy Services

Services include: 

• Guiding the Client in engaging with a Financial Management Service (FMS).

• Coordinating and sourcing agencies, companies and/or staff to fulfill the needs outlined in the PCP and IPP. 

• Facilitating the implementation of the spending plan, including vendor integration with the FMS. 

• Providing continuous support and advocacy throughout plan execution.

• Managing monthly SDP budget reconciliations with the FMS. 

• Handling vendor onboarding, management, and any necessary adjustments to the spending plan. 

• Addressing any new needs that arise and amending the budget accordingly.

• Conducting annual reviews and updates of the PCP. 

• Coordinating and supporting the annual SDP IPP meeting. 

• Assisting with transitions between regional centers, if needed. 

• Reviewing and reconciling spending plans with FMS reports monthly.

• Supporting annual IPP meetings and budget recertification processes.
