Hammond Facilitation is committed to upholding and protecting the rights of people with disabilities – intellectual, developmental, or physical. People with disabilities have the right to self-determination and are entitled to the freedom, authority, and support to exercise control over their lives.

Mission

We empower physically, intellectually, and developmentally disabled (P/I/DD) people and their families to discover and identify their hopes and dreams. We believe every individual has the right to a self-determined life.

Strategy

With support and encouragement, we work as a team to design a Person-Centered Plan, for P/I/DD people while creating a sustainable circle of support and maximizing successful strategies. We facilitate entry into the Self-Determination Program and design a spending plan which allows the participant to effectively utilize their funds.

Purpose

Through Person-Centered Planning and Independent Facilitation, we strive for every person to live their best life.
